Request concerning choice of name (RF-1452)

The chosen name must be registered before the child reaches six months of age.

The person or people who have parental responsibility for the child must notify the Norwegian Tax Administration of the name that the child is to have.

Parents with joint parental responsibility must agree on the choice of name. In the case of shared parental responsibility, a completed form will be automatically sent from the mother to the other parent for approval and signing. The sender will be notified when the other parent has submitted the form.

If the sender is registered as having sole parental responsibility for the child, the form will be sent directly to the Norwegian Tax Administration for processing.
